I haven't seen one that had any significant amount of gunk in it. Maybe if it had been misfiring and one cylinder was really full of oily soot it would make sense to clean out the manifold -- or I might be inclined to think it would just burn out once then engine was running well. Usually when an engine's in that bad a shape, the valves (or at least the exhaust valve on that cylinder) will need work because the gunk has gotten under the valve. If you didn't have to do any valve work, then I would tend to think everything downstream of the valves is fine. The main reason I find for pulling the manifold is the heat riser being rusted in position or clogged up with soot to where it can't move. Might be something you'd want to check. If the heat riser moves freely, and the valves look good with the head off, and the manifold is not cracked or leaking, I can't see any particular reason to do anything more to the manifold.
