Mark,
The first nut to go on after the bearing,
Is the nut with the pin on the face.
After you adjust the bearing,you move the nut back just enough to line up with a hole in the lock washer,with the tab in the spindle grove.
Also if you flip the washer over it changes the hole location a little.
Then install and tighten the outer nut securely.
