I deal with glass regularly. Heat stress is a common failure of glass, and Tempered glass has a higher failure rate than other types. Nickel sulfide occlusions are ever present during the process of manufacturing glass. They expand and contract at a different rate than the rest of the glass and eventually penetrate the different surface tension layers of the glass causing it to fail. Another source is when the glass is not edged. Raw, or sharp edges, fail very easily because for the same reason, you are affecting the surface tension layer. Be sure all glass has sanded, eased edges at the very least.
Glass is glass. It breaks. Costly when it does, but its glass. There are so many things that can cause it to break.
