I don’t believe that yours is OEM but I may be wrong. The PTO from the factory would have been dual output front/rear. I actually put a M37 PTO on my NP420 transmission when I removed the nonsynchronized transmission. It fits just fine and is basically identical to the civilian PTO except no rear output. I believe it is the same casting with a shorter output shaft and the rear cover/handle attachment doesn’t have the hole and seal for the rear output shaft. The civilian shift handle would possibly work on the military PTO but not the military one on a civilian PTO. This is because the civilian shift handle is bent to go around the rear output shaft.

All said and done, just keep your current PTO if it is working. These trucks are tall enough that your not going to drag it on anything. Secondly, PTOs are not cheap, so why spend money on something you already have that works.
