Around here code wants insulation in the crawl space above the walls but does not require it under the floors. We did the whole floor after living with no insulation for a decade - it made no difference. I have installed foam blocks above the walls in other homes and the customers were fine with it. All the plumbing and hot water tanks are in the crawl space. I think they provide enough radiation to keep things from freezing.
The crawl space is skim coated - in some places gravel or sand is allowed but i refuse to live in a house without a slab underneath.
