The other guys all have good ideas, but I've got to add a bent disc to the equation. Some of the rebuilts come with bent hubs, when I worked in a shop with a brake lathe I would mount each one on the lathe to check for run out. And you can bend them when installing if you let the tranny hang from the clutch hub while wrasslin the beast into alignment. The bent disc will feel like it's releasing all right, but will still rub on the flywheel- maximum release is about 1/2 inch, if the disc is bent 3/4, you'll have problems.
The best way to install a tranny is to put LOONG studs into the bolt holes in the bell housing, then simply slide the tranny home, keeping it parralell to it's mating surface, If it won't slip right in, try stepping on the clutch while cranking the engine. If it's all lined up, the 'bell' housing might even 'ring' when the transmission clangs into place.
