Check the compression then put a vacuum gauge on the engine it will tell you a lot. It will tell you about timing, valve condition (sticky valve). Watch the gauge when accelerating, at idle and at higher rpms. Look at the link below about vacuum gauge tests. A vacuum gauge is the cheapest engine diagnostic tool you can have, but it can give you so much information to help you.
The guys here can give you lots of ideas as armchair mechanics, but you need to run some tests as you are the one with the truck in your garage. Try the vacuum gauge test then ask more specific questions when you have some results.
