A hesitation , indicates that the carb needs an adjustment to one of it's squirters . Noise is relative - may be that is the normal noise for the beast or that the trans needs rebuilding .
But these things need constant tweaking , usually minor in nature but these trucks were built in the age when an engine would get rebuilt on average around 67 thousand miles .
Not like today when 180 thousand miles is quite common .
