Can't answer question 1 unless the nuts are being used as shims. Q2 - 2 hardwood blocks with a 1/4" reinforced rubber or canvas pad on top, same pad but no block at the back. 5/8" bolt on driver side front tightened up solid to locate the cab, other 3 bolts are 1/2" with springs to compensate for any chassis flex. Do up all your bolts to locate cab before you tighten the steering clamp under the dash - steering column not designed to support/locate cab.
