Honestly, the only real difference is going to be between brands of hitch. Since you have a flatbed, you would probably want to go with a gooseneck. The 5th wheel hitch takes up alot of room, even in a regular pick-up bed; you have to unbolt the hitch from the mounting rails to utilize the bed space, and even then the rails are still bolted to the bed.
If you pull a flatbed/toyhauler trailer, go with the gooseneck. If you pull an RV trailer, go with the 5th wheel. There are couplers to adapt to either if you have both trailers, as well as the cushioned couplers which utilize an "air bag" cushion. I put a B&W gooseneck in my 2007 Dodge 3500 dually, installed myself and I am very satisfied...
