I think I have the levers and linkage in pieces, but it's nothing you would want to use. They torched a big hole in the floor just ahead and to the right of the seat, mounted a rod across it using a generator bracket (the one good part of the deal), and hung 3 levers on it that were just 1/4 x 1" bar stock with pieces of iron pipe in between them as spacers. One had a long pipe "cheater" attached to the top of it. The bottoms of the levers had some rods with threaded adjustable ends to connect to the PTO and gearbox, but they were all rusted up and unusable. I think I threw them out. You could have them if they're still around, but I don't think they'd do you any good.
It wouldn't be hard to replicate that setup, but your truck is too nice to torch a big hole in the floor pan. I'm not a big fan of cable-actuated PTOs because they always seem to feel kind of mushy, but I expect that when they were new and the control cables were clean and slid well, they worked a lot better.
For the amount of work it would take to fabricate a rigid linkage, maybe it would be easier to just clean up or replace your control cables and get that system working right.
Does your rear winch have a drive sprocket that matches that gearbox? What do you think happened to the chain? I will have to crawl under my telephone truck and see how they handle that situation because it has dual drum winch and cable spool drive in the rear, and an auger drive PTO in the front bumper, so they must either have a double-ended PTO on the tranny or a splitter box like this. I haven't gotten into working on that truck at all yet.
