I believe those are called "Budd" nuts. Used with Budd wheels. Budd was a truck wheel manufacturer. I found a similar lug nut at a truck parts supply house recently. They are made in several lengths. I have never seen them with that long unthreaded area. Seems to be a spacer. You may be able to buy a Budd nut and then make a spacer that would match the other four. Just be carefull to make the spacer the same length or the wheel may wobble.
