Kaegi pretty much nailed it, seen it before, stud strips out in the mid section because the end thread was damaged and at some time someone tried to get the nut off and it wouldn't come. If you have free access, sometimes you can cut it off with an angle grinder and knock the last bit out, common fix then is to tap it same size and screw a set screw from the inside and weld in place. Sometimes have to grind part of the head off that set screw for clearance.
