That's awful. I've had the same thought - one minute all you have is a mechanical problem, but a bad decision or some bad luck and instantly you're in deep trouble.
There's a cautionary tale in my area about a guy that took his arm off at the shoulder with the winch on his wc. Glove caught on the cable. He did everything wrong - working alone, no kill switch up front, and getting too close. Just like I've done a bunch of times...
It's easy to underestimate the risk.
