Here is a Ford Belt Pulley to match the PTO Shaft I have listed. Just bolt them together and put a Universal joint on the end of the shaft with a adapter plate to match the bumpers hole. My shaft will fit inside this and match up and the Bracket on the mounting end (Belt Pulley) will go around the PTO shaft housing. So all you need to do is bolt both to a flat plate drilled out for both sets of holes and drill out the flat plate to match the back of your truck. You would need 1 flat 1/4 inch thick plate about 6 inches square with a aprox 3inch hole in the middle for the PTO Shaft housings shaft to go through bolt it to it, bolt the Belt Pulley around it, And drill the plate to match your trucks holes. Now all you have to do is build a drive line to go to the PTO on the Transfer case and a machine shop could weld you a shaft up for around 100.00 if you had the ends for it or just re/work a old drive shaft with a slip joint on it to make it easy to install. The belt pulley listed should sell for around 125.00 to 150.00 here is your chance. Mark H.
