I was thinking that fields should be available for:
name
email
address
phone
and a notes section for truck info and the other things that have been mentioned.
People could fill out the whole form or only the parts that they were comfortable with disclosing to the public domain. I for one would disclose my phone number because I figure that if anyone associated with the forum wants to get a hold of me then what is the harm. In addition, I can handle the responsibility that comes with it. Others may not feel the same way.
