I just whipped this up. PHP using MySQL as the back end database. I used PHPMyEdit to create the form.
I'll have to shrink down the size of the text and boxes to fit more stuff in.
It works. Feel free to test it. You can sort the columns by clicking on the word at the top. Search is done by clicking on the "V" on the left side.
