Intermittently the clutch was not able to fully disengage. I believe a port in the master cylinder's bore was blocked at random times, not allowing sufficient fluid to be ahead of the piston.
When the clutch did not disengage, the input shaft was still being driven somewhat, but not so much that a synchro could not bring it to a halt before locking a gear to a shaft.
All that "work" ruined several synchro rings, as I would use various gears to get the thing stopped. Sounds abusive, I realize, but you have to do something if you can't get the dealer to fix it the first time.
