It wasn't a flat fender but we had one on a automatic truck. The pto unit was mounted on the side on the transfer case. You had to put the transfer in Neutral and use the selector for engagement. Had a rear pto as well but had to flip the unit around as it wasn't 2 ended like flat fenders have. Not as handy, but it did work.
