I have a power wagon with dual wheels. They do not use longer studs.
You install your normal wheel first. Then instead of using the normal lug nut you install a special lug nut about 2" long. The special lug nut has internal threads and external threads and a 7/8" square on the end. The special lug nut holds the inner wheel in place. You tighten it using a wrench with a 7/8" square socket.
Then your outer wheel is installed. The outer wheel is different than the standard wheel because it has a special offset to give the separation between the 2 tires.
Finally the standard lug nut is installed to hold the outer wheel on.
I realize this is a poor written description. Next week I will take the parts to work and take some digital pictures as I have seen this question asked several times.
These parts may not be too hard to find. My dad had a 48 2 1/2 ton truck and he gave me the old lug wrench he still had from the truck. It has the 7/8" square on one end and the 1 1/2" end on the other end. These trucks all had dual wheels so there may still be some around in junkyards.
