Each firing of the combustion causes the crankshaft to twist, thus causing vibration, balancer helps control that. Compare the flywheels, trucks may have heavier flywheels which help to counter torsional and resonance vibrations from the crankshaft, thus not requiring a balancer. Also the rubber in your balancer may have become hard over time, reducing its ability to control these vibrations, something to be aware of.
