so to answer my own questions...
1. Yes, the body of the limiter has to be grounded so the heating coil draws a current to heat the bimetallic strip.
2. No, there doesn't need to be a load. The output voltage is regulated by a bimetallic strip that heats and cools cycling the output on and off causing a chopped DC output that averages somewhere around 5V.
My disassembled voltage limiter had a burned up heating coil causing the points to remain closed all the time and thus passing 12V continuously to the output terminal. I read somewhere that these guages are rated for 6V.
