I had an engine knock that only appeared as the engine was under a heavy load such as going up a grade. I had checked rod clearance all was fine, I just wasn't able to determine what it was. The engine head gasket failed so I pulled the head and looking at the pistons, number 5 looked a little funny, so I decided to pull the pan and pistons. The number 5 piston had broken rings and two piece had work parallel to the piston and was causing the knock at the bottom of the stroke by logging against the cylinder wall. Go to the temp section to see a picture. Engine ran fine, didn't smoke, just had that knock.
