can pto be installed with trans in the truck

i pulled the cove off the transmission and was going to install the pto i have but being a little heavier than i thought it doesn't seem to maneuver in.... and if i got it in how do i set the backlash. on a normal pto u can pull the little 4 bolt cover and set it with shims ( gaskets) . you already have the backlsh from the pto gears hor would i tell what it would be from the trans gear to the pto gear..
