Plumbing is the first and biggest thing to check. Make sure you are going from the pump to the HB then through the PS box and are using the right ports on each (three on the HB and two on the PS box).
The other big problem for a lot of people is the return line from the HB unit. It needs to have near-zero back pressure, which ideally means having its own return port in the reservoir/pump, but a Y connection is supposed to work as well (some have had issues with a T fitting).
Assuming the unit is working properly then I'd guess it's the low pressure return line isn't low pressure.
Otherwise how did the peddle feel? They were standing on it but was that because it was soft and they had to press really far, or was it rock hard and they couldn't get it to move? It could be the master cylinder or proportioning valve. What size is it? Is there a proportioning valve?
