Stack up is as follows:
Flat washer
Lock washer (split or star lock washer)
Hex nut
The flat washer is there becuse the hole SHOULD be oversized to allow for misalignment and fitting. Without the flat washer the split lock washer would "jamb" or even fall thru the hole.
You do not use a lock under the head of the bolt UNLESS there is a permanently fixtured (welded or pressed in) nut in the adjoining metal piece.

In a nut shell a split lock washer is designed to keep the bolt or nut from vibrating loose so it must be up against the bottom of the head of a bolt or a nut
