Water is returned to the top of the head, roughly 1/4 of the way back. As mentioned the metal pipe has a very exact round sleeve that when pressed or driven into the port on the head is water tight. Also on the insertion end is flats machining so a driver can be used. When you make the insertion, believe me your not going to pull it out without destroying it.
Can you plumb it elsewhere. Maybe, but I believe that Cummins in the design of the 4B Series engine spent millions of dollars in the total design. Who am I to second guess Cummins and do it different.
