I would almost think (looking at the photo blown up) that the shaft / thrust washer teeth failure was due to not seating, or cracked on the shaft when assembled, and once the washer came apart, needle bearings were flying fast at hwy speed , I've done it but was lucky enough to feel it , with out having the Parts in my hands, I can't comment on alignment, and/or lubrication
