It certainly requires a new bellhousing from Dodge, most likely one for a (recent) 360/5.9 V8 to NV4500. This particular setup used the "standard duty" NV4500 with 1-1/8" input shaft. The bearing reatainer snout on the 1-1/4" version has an OD of ~1.73" whereas the standard duty is 1.433". This could relate to the throwout bearing chosen.
You should call Advance Adapters at 800-350-2223 and probably order their very informative NV4500 conversion manual.
I thinks it's possible but there's some "fiddling" to be done.
