There are a couple ways to attach the pump to the crankshaft, either directly or through a simple clutch. I don't know what specific hardware is used, but if you want I'll be happy to ask the guys at a local fire engine builder's shop what they use. Pumps are still front-mounted on vehicles today, so the parts are available
One problem with a direct connection to the engine is that the pump spins all the time. The pump seals are "lubricated" with water, and if the pump is run dry it damages the seals. If the pump always has water in it, it has to be circulated throught the tank, which robs the engine of quite a bit of power. If the water is not circulated the water trapped in the pump will absorb enough kinetic energy to cause a steam explosion.
