I am pretty sure you have the wrong box in the truck. someone probably replaced it with a boneyard box from a 1.5 ton. maybe it was only appearing to be right when you pulled it out because the installer jammed it over into place. that is what happened on Johns truck.
Can you measure the offset and we can compare? I measured a 4x2 1/2 ton box and it was 5 3/8. but I am not sure that would be the same as a PW
