Smoking after starting and smoking going up hill may have two different causes.
Smoking after starting is normal in worn engines until the engine has a chance to heat up and burn excess oil.
Smoking going up hill may be just because of the extra load imposed by the hill, but it sounds like it is giving up the ghost. I suspect, as others have said, that something like a hole in the piston or a head gasket failure is happening.
Has the smoke changed colors or it the same color all the time? Have you checked the coolant level?
