I used NAPA number SKF N4008-SA clutch kit which included all the parts you mentioned. The flywheel pilot I used is Cummins/GM 6302-2RSJ. The Clutch Slave I used is NAPA 360027 with a bore of .787" and a Clutch Master from NAPA # 35085.
