Dualies were an option on the rear axles, and even on the front axles of a few early versions, probably prototypes.
In normal usage, the dualies apppeared only on the utility bodied trucks.
Don't know what bolt holes you are referring to. By any chance are they in the shape of a teardrop style fender? If so, this may be a converted civilian bed. The civilian beds did not have a stake pocket over the fender, while the military fender did.
