If the rod is broke or severely twisted or bent, it requires replacement. Check the connecting rod with a connecting rod aliner. If the rod is bent or twisted more than 0.0015 inch per inch of length, use a connecting rod press to straighten rod. This does require the assistance of a machinist to do. Due to the rod off-set, rods do get out of alignment over time.
