Do you have a 2WD or 4WD 4500. If 2WD, how long is it? From front face to center of U-joint for rear yoke.
"Easiest" way would be an adaptor plate. With countersunk holes for Allen head installation screws to bolt to stock pattern, and threaded holes for NV4500 mounting bolts. With bearing retainer index hole for 4500 and 4.75" indexing ridge for stock bellhousing concentric to the nearest 0.0001"!! So it'll need to be an inch thick. So you'll want the "HD" Dodge/Cummins version which has a slightly longer input shaft. Even so you'll need an extended pilot bushing, probably.
They do make such an animal for adapting the NV4500, HD Dodge version, to Ford bellhousings. The center index ridge circle might be a hair bigger for Ford, it could be turned down to 4.75". And using a spare bellhousing a machinist could make the countersunk mounting holes easily enough. As long as they don't overlap with the pre-existing Ford holes. Maybe you could buy one before the Ford holes have been installed.
The Ford item has a indexing diameter of 4.848", so only 0.049" needs to be taken off the outside of the ridge, between 1/32 and 1/64. You need to know the distance between the LOWER pair of Dodge holes and make sure the adapter isn't so "hourglass" in shape that there's no metal there to put the lower holes in. Old Dodges have the lower holes only 2.75-3.5" below the upper holes; most new patterns have 5-7" vertical separation.
