You pictures didn't show up but assuming its as other have suggested, it's a common problem that can be a real PITA to get apart. Good tire shops have a special wrench that goes over the outer (female) nut and has a hooked end that goes into one of the holes in the wheel. Then you can use an air wrench on the square (inner) stud/nut and screw it IN to separate the assembly. If you have a 1-1/2" box wrench you might be able to use that, with the open end jammed in a wheel hole, and an impact wrench on the square end, but it if it's really and you're using a 1" impact wrench you're liable to bugger up you box wrench.
This should not have happened with a single wheel. Usually it happens when you take off the outer dually. In that case, the field expedient is to put it all back on, tighten down the other lug nuts, and remove the stuck outer lug nut. When you do that, the stud/nut should be held in place by the tightened-down wheel. The only time I've had this problem is when the actual stud in the brake drum breaks off.
If you have extra lug nuts or your time is worth more than your money, it's sometimes easier to torch the old one assembly off, taking care not to gouge the wheel. If you don't run duallies on any of your rigs, you have no use for either an outer bud lug nut or the stud/nut, so you might as well torch it apart. Either way you'll need to buy one new inner budd nut.
