Out of curiosity, what does it go in? If it doesn't have to be incredibly precise (like for an old vise) any competent machinist should be able to easily make one on a lathe.
A semi-standard machine parts outfit like Misumi might have a customizable part you could use. They have a catalog of standard simple parts that you can customize in various limited ways and which they then make to order.
Depending on the application, can you swap out the existing non-standard screw for a common standard one and replace the nut at the same time? Everyone who sells acme threaded rod also sells the special nuts to fit it.
