6 speed was only worth it because the Getrag was useless & I got a factory new NV5600 off Ebay for $2000...which was basically the price of an NV4500. I'd take the NV5600 over the NV4500 but they're both excellent.
I wouldn't pay extra for the extra gear, if that's what you're asking. In my application, it was my red 3500 4x4 dually & I was towing tons with it & so the 6 speed definitely helped in the mountains when it was stock. Once I fancied the motor up I could just skip every second gear :)
