Do you want the ladder to be always attached? Or are you going to fold it up and hold it secure?
I'd weld a piece of 1" pipe (square/round vertical at the bottom of the steel sitting on the hinge. Then create a similar shape (vertical) and weld it to the heavy cross-member under the tailgate/bed sheet metal. Basically like a stake bed, except externally.
You'd have to play with the geometry and create a "stop" in the vertical piece attached to your ladder as you have to "shim" is because of the vertical difference between the two locations...
You could use just a vertical plate of steel say 2"x3" and create a narrow slit (like a stake pocket) on that cross-member if you wanted it less obtrusive for when the ladder isn't there...
