I imagine you have examined the shaft to see if it is scored or otherwise damaged.
You will be replacing it again, it seems. Have a good look [as best you can under the difficult circumstances] at the bore into which the seal fits. Did you put non-hardening gasket sealer on the OD of the seal before you installed it?
