I can answer your questions about the requirements that the U.S will impose(forms,etc...) The most important factor is the year and what type of fuel that it uses (different rules for diesel depending also on the year).
The requirements for getting it out of Canada, I cannot answer. Yes, you have to satisfy both governments.
What is the year and type of vehicle?
