Your end is worn, too pointed.
Also your slot is rounded out. Weld in new metal to fill up the rounded area, then grind and file the slot back to a rectangle.
You may be also able to weld metal on the end to widen it out and not be so pointed.
See the pic I shared from my Image Event site. The ends are "good". One ball is bad and the other only has slight wear. I ended up fixing both by adding metal to get them back to a nice rectangular slot.
