That torque spec is roughly a big guy standing on the end of a 2' bar, so perhaps a big guy pulling as hard as he can on the end of a 2' bar isn't quite enough.
We've had a lot of trouble on the Mine Tour trolley last summer with the budd lug nuts coming loose. The rear dualies are hidden under a skirt so it's hard to check them. There was no excuse for the drive not noticing the front ones while I was gone for a week, with the result that we ruined a wheel, a tire, and a set of wheel studs. They run 9.00-22.5s on 6-lug rims. I never put anti-seize on the studs, but I only hand-tighten them because I need to be able to change a tire at the office if need be.
So far, we've broken 3 rims due to the lug nuts working loose, the wheel wobbling around, and cracking the rim. It's a real puzzle because we only put about 700 miles on the trolley per season, while I've gone thousands of miles in my M35s with 6-lug budd rims and never had a nut work itself loose. We eventually set up a policy where one of the miners removes the skirts and tightens all the nuts every 2 weeks. After doing that diligently, by the end of the season they seem to have stabilized and are not working themselves loose any more. Next summer I'll just have them check once a month.
