One shop I worked at had a worm gear pivot on the engine stand, sure worked nice. I've had too many close calls of nearly tossing an engine on the ground when rolling it over on a cheap stand. I do see what looks like a long screw thread for balancing the rig, but I sure hate the thought of losing balance and having a cab smack the floor. Or like the engine stands try to do, the weight gets moving and the whole stand flips. I've got a worm and gear that size, just waiting to need a heavy duty engine stand. A right angle reduction gear might be easy to rig too.
