They often come loose when somebody has been a bit heavy handed when fitting it. If you drive it down too hard it squeezes the end and makes it loose. You can stretch it back using a piece of solid round bar in a vice as an inside dolly and tapping around it with a hammer. I use a fast drying hard setting sealant like permatex when assembling. Also some filler tubes have a clamp on support that bolts to the engine and these are good, but you still need to seal the tube properly. I have also seen them fitted with an 'O' ring that is between the block and the bulge, but no use if the pipe is loose.
