Nick,
What you have is someone's modification. There should be no caged nuts or flange on the filler neck.
The gas tank and the filler neck are connected with a big fuel resistant rubber hose with hose clamps on each. Yes, there is a big a$$ grommet that goes over the filler neck and fits the hole in the bed side. I bought the grommet from VPW and the hose from my local automotive parts store.
