May be a W-300, or perhaps a W-500. I cant be sure from the photo, but it looks good.
Go to: "www.firetec.com"
Then select the following:
"Site Index",
"ANTIQUES"
Then find vehicle "AQ-4675" and click on it. A picture and some other information appears.
Im not sure of the location, as FireTec is a used vehicle clearinghouse.
