Lock washers on head bolts?

That's an absolute no-no. You should never use a lock washer on ANY torqued bolt on an engine (oil pan, oil pump, intake, head bolt, rod bolt, whatever). Just two of the myraid reasons: On any rotating part, the massive reciprocating forces eventually will spread the washer to the point of slinging off of the bolt, TORQUED OR NOT. As bolts heat up, they stretch. The expansion is even more pronounced on the washer, as it is sprung. Another reason is that because it IS a sprung washer, it throws the torque reading off when installing any fastener with one on it. A properly torqued bolt with a flat washer will never come loose as long as it doesn't have a damaged thread-mating. Just because you or someone you know has done it with no ill consequences, thirteen other guys did it, grenaded the engine/the part fell off, and didn't tell anyone. There's a very good reason there's never been a lock washer installed on any car from the factory- they're not supposed to be there!
