I would support any PTO driven shaft that was 48" long or longer, even if it doesn't change direction. They turn so fast they can start whipping and cause lots of problems. A pillow block/carrier bearing will do the job. I would suggest one of the yokes be about 4" long with a key way to allow some movement fore and aft.
